We are so excited you’ve been invited to an Extreme Birthday Party!

Please complete this form using your information (the parent). Enter the birthday party host’s email or phone number in “referred by” & select their name.

For Desktop: Add each invited child as a “relationship” to sign a waiver for each.

For Mobile: Add children’s names & birthdates in this form (if multiple separate by commas). We will add an account for each child & attach them to you. Once you receive a welcome email for the children you can log back into the app, switch to their profile/s in the top right corner and accept their waivers.
